Registration and ONE ID: Frequently Asked Questions

The following information answers many of the questions primary care physicians commonly ask about registration and ONE ID.

ONE ID Login and Registration

What is ONE ID and why is Ontario Health using the ONE ID application?

ONE ID is an identity and access management service with robust systems and processes that enables health care providers to access eHealth services in a trusted and secure manner. ONE ID is part of a province-wide electronic health strategy to reduce the number of IDs and passwords a user must manage and remember to access services. In the future, your ONE ID identification and password will enable you to access a growing variety of provincial data sources and reports. How do I complete my registration if I forget my temporary password or if it expires?

Upon registering for ONE ID, you will be provided with a temporary password. You will be prompted to change your password when you log in for the first time. If you have lost or forgotten your temporary password, you can follow these steps:

  • Go to the MyPractice Primary Care Plus (with Screening Activity Report data) page.
  • Select the "I forgot my ONE ID password" link.
  • Enter your login ID (firstname.lastname@oneid.on.ca) and date of birth.
  • Next, you are prompted to verify your identity by answering a set of challenge questions selected at the time of your registration. Note: answers are case-sensitive.
  • Create a new password.

If your temporary password has expired (90 days after issue), you will not be able to reset it online. New users whose temporary passwords have expired will need to call Ontario Health at 1-866-250-1554 for assistance.

What should I do if I have forgotten my ONE ID login or password, or my password has expired?

Go to the Locate your account page to locate your login ID or reset your password.

ONE ID passwords are valid for 12 months before expiring.

Authorizing a Delegate

As a physician, how can I authorize a delegate (e.g., clinical staff member or office administrator) to access my MyPractice Primary Care Plus (with Screening Activity Report data)?

Physicians should send a registration request to oh-servicedesk@ontariohealth.ca on behalf of their delegate. The Ontario Health service desk or Local Registration Agent (LRA) will schedule a registration appointment with the delegate(s) to set up a ONE ID account for them. Registration appointments will be scheduled based on the location and/or date of request.

Once a delegate is registered with a ONE ID account, physicians must appoint them as a delegate to view their individual report. To appoint a delegate, visit eHealth Ontario, log in, select "Manage your delegates" located at the bottom of the screen, and then search for the delegate's account by their login ID.

The MyPractice Primary Care Plus (with Screening Activity Report data) includes patient personal health information. As a health information custodian, you have an obligation under the Personal Health Information Protection Action, 2004 (PHIPA) to protect that information from unauthorized collection, use and disclosure. This includes ensuring timely revocation of access to the report by people who should no longer be your delegates (e.g., they no longer work in your office).

Troubleshooting

What should I do if I’m having trouble viewing my MyPractice Primary Care Plus (with Screening Activity Report data)?

You may have difficulty viewing the report if you are not using current versions of your operating software or web browser. The report is optimized for access on recent Windows and macOSX desktop operating systems, as well as recent versions of Internet Explorer, Mozilla Firefox, Google Chrome and Safari. You may have to update to a more recent software or browser version to view the report.
