Guideline Objective

To address the needs of CMH patients, Cancer Care Ontario undertook a quality improvement initiative to support timely access to high quality, coordinated CMH services, guided by the following core objectives: - Ensure patients have timely access to high quality care in appropriate settings, as close to home as possible; - Support providers to collaborate and align around best practices, and have a manageable workload; and, - Optimize use of healthcare resources.
