Why is the Screening Activity Report only available to patient enrolment model and Sioux Lookout municipality and zone primary care providers?

The cancer screening data we collect is associated with a patient’s enrolment status. This information can be easily collated for physicians who have enrolled patients.

In addition, we are able to extract screening data for non-patient enrolment model physicians servicing the Sioux Lookout municipality and zone (27 First Nations) using postal codes that have been confirmed with these communities. This allows us to develop Screening Activity Reports specifically for those communities.

There are significant gaps in the data for patients who are not enrolled with a physician, and it can be difficult to determine which physician a non-enrolled patient belongs to.

We are exploring opportunities to provide Screening Activity Reports to more primary care providers who do not practice in a patient enrolment model.

The report is exclusively online to keep it safe, secure and accessible at any time in a user-friendly format.

How the Screening Activity Report Differs from Other Reports and Records

What is the difference between the Screening Activity Report and the Target Population Services Report?

Similar to the Ministry of Health and Long-Term Care’s Target Population Services Report (TPSR), the Screening Activity Report (SAR) from Cancer Care Ontario provides patient enrolment model and Sioux Lookout municipality and zone physicians and nurses with a summary of breast, cervical and colorectal screening activities. The SAR uses many of the same data sources and calculations as the TPSR, but provides additional information, including:

  • a visual dashboard providing a quick overview of your enrolled population's screening activities for all 3 screening programs
  • comparisons of your screening rates with those of peers in your Local Health Integration Network and the province
  • detailed lists of patients and their screening status, including screening test results and follow-up procedure dates

Are there benefits to using the Screening Activity Report if I already use my electronic medical record for cancer screening?

Yes. The Screening Activity Report can aid your electronic medical record-based cancer screening activities by providing you with:

  • a double check of patients who may need screening or follow-up
  • historical screening data that may not have been migrated to your electronic medical record
  • clear, actionable groupings of patients to support population-based preventive care for cancer screening

ONE®ID Login and Registration

What is ONE®ID and why is Cancer Care Ontario using eHealth Ontario’s ONE®ID application?

ONE®ID is an identity and access management service with robust systems and processes that enable healthcare providers to access eHealth services in a trusted and secure manner. ONE®ID is part of a provincewide electronic health strategy to reduce the number of IDs and passwords a user must manage and remember to access services. In the future, your ONE®ID identification and password will enable you to access a growing variety of provincial data sources and reports.

 

How do I complete my registration if I forget my temporary password or if it expires?

Upon registration with ONE®ID you are provided with a temporary password. You will be prompted to change your password when you log in for the first time. If you have lost or forgotten your temporary password, you can follow these steps:

  1. Go to the Screening Activity Report page.
  2. Select the "I forgot my ONE®ID password" link.
  3. Enter your login ID ([email protected]) and date of birth.
  4. Next, you are prompted to verify your identity by answering a set of challenge questions selected at the time of your registration. Note: Answers are case-sensitive.
  5. Create a new password.

If your temporary password has expired (90 days after issue), you will not be able to reset online. New users whose temporary passwords have expired will need to call eHealth Ontario at 1-866-250-1554 or email their service desk at [email protected] for assistance.

 

What should I do if I have forgotten my eHealth Ontario ONE®ID login or password, or my password has expired?

Go to eHealth Ontario’s Locate your account page to locate your login ID or reset your password.

If you are having difficulties, please call eHealth Ontario at 1-866-250-1554 or email their service desk at [email protected] for assistance.

ONE®ID passwords are valid for 12 months before expiring.

How do I edit my eHealth Ontario ONE®ID profile?

Log in to your ONE®ID account to access the profile management application where you can change attributes of your profile.

Authorizing a Delegate

What is a delegate and what are the advantages of authorizing a delegate?

A delegate is a member of your primary care team (e.g., a nurse, an office manager or a clinic staff member) who is authorized to access your Screening Activity Report. A delegate is set up with their own unique ONE®ID account and is given permission from a physician to download and view the report. By authorizing delegates, the tasks involved in cancer screening can be shared among team members.

As a physician, how can I authorize a clinical staff member or office administrator to access my Screening Activity Report?

Delegates must first be registered with eHealth Ontario by sending an email to [email protected]. Registration appointments will be scheduled based on location and/or the date of your request.

To appoint a delegate, visit eHealth Ontario, log in, select "Manage your delegates" located at the bottom of the screen, and then search for the delegate's account by their login ID. For more assistance, please see the video on ‘Appointing a Delegate in ONE® ID’ located under the Support Videos and Info’ tab.

The Screening Activity Report includes patient personal health information. As a health information custodian, you have an obligation under the privacy law to protect that information from unauthorized collection, use and disclosure. This includes ensuring timely revocation of access to the report by people who should no longer be your delegates (e.g., they no longer work in your office).

Using the Screening Activity Report

How do I view and print my Screening Activity Report?

You can access your report from the Screening Activity Report page by clicking the Access Your SAR button:

To print your reports, select “Actions” and “Print” from the report navigation bar.

Before you print the report from Excel, you will first need to set your printing preferences (e.g., size, layout, page orientation) to accommodate the report's size.

How do I change the sort order of the report to something more meaningful?

Within the Screening Activity Report application, any column with a bi-directional arrow beside the column header can be clicked to reverse the sort order. By default, all detailed reports are sorted by patient surname.

How can I quickly view patients who are overdue for screening or require follow-up of an abnormal cancer screening results?

There are 2 ways to skip directly to this information:

  • From the dashboard, navigate to the patient population of interest and select the number associated with the screening status and program. This will take you to the list of patients in question. For example, to see a list of rostered patients who have had an abnormal fecal occult blood test with no known follow-up, scroll across the row that reads “Abnormal screen, follow-up needed” within the “Colorectal Screening” column and select the number.
  • You can also view each of your program reports containing all eligible patients by clicking on “Breast Screening,” “Cervical Screening,” or “Colorectal Screening.” To filter, scroll to the right and click on the grey, vertical navigation bar. This opens up the filtering menu. Select the sub-statuses from the drop-down menu to filter by a specific cancer screening need.
