Screening Activity Report (SAR)

The SAR is an online tool available to primary care physicians who practice as part of a patient enrolment model, or to non-patient enrolment model physicians and nurses who practice in the Sioux Lookout Municipality and Sioux Lookout Zone (27 First Nation communities). It provides screening data for breast, cervical and colorectal cancers to help physicians improve their cancer screening rates and appropriate follow-up. The report platform is interactive, allowing physicians to quickly find specific cancer screening information for each patient.

A Screening Activity Report (SAR) is available to the following people:

  • All patient enrolment model primary care physicians with enrolled and screen-eligible patients
  • Non-patient enrolment model physicians and nurses servicing the Sioux Lookout Municipality and Sioux Lookout Zone (27 First Nation communities)

Patient enrolment family practice models include, but are not limited to:

  • Comprehensive Care Model
  • Family Health Organization
  • Family Health Group
  • Family Health Network

A Sioux Lookout and Zone SAR is available to the following groups:

  • Non-patient enrolment or alternative funded model of primary care for the Sioux Lookout Municipality and Sioux Lookout Zone (27 First Nation communities)
  • The Hugh Allen Clinic Family Health Group
  • 27 First Nation communities serviced by the Sioux Lookout Regional Physicians Services Inc.

Information Provided in the Report

The SAR includes the following reports:

  • Dashboard – 1-page cancer screening summary and graph comparing a physician’s screening rates with other physicians in the same Local Health Integration Network and the province.
  • Enrolled and Eligible Patients – An integrated view that tells physicians the breast, cervical and colorectal screening status of all of their enrolled patients ages 21 to 74.
  • Enrolled Patients by Program – Provides the cancer screening-related history for all eligible and enrolled patients for each program:
    • Breast: enrolled patients (ages 50 to 74)
    • Cervical: enrolled patients (ages 21 to 69)
    • Colorectal: enrolled patients (ages 50 to 74)

The information in each report is updated on the 10th calendar day every month to provide more timely screening data.

All of the reports can be exported to a PDF or Excel file, with the exception of the dashboard, which can only be exported as a PDF. When accessing the data to perform a task, physicians should export and save the report to their computer. The old report is always replaced by the most recent version at the time of refresh.

The Screening Activity Report uses eHealth Ontario's ONE® ID to authenticate users.

To register:

  • For patient enrolment model physicians and Sioux Lookout Municipality and Sioux Lookout Zone Physicians and nurses: send a registration request to ONEIDBusinessSupport@ehealthontario.on.ca.
    • Please include your name and College of Physicians and Surgeons of Ontario (CPSO) number (or the CPSO number of the physician you support) or nurse registration number (ONA) in your email request

Registration appointments will be scheduled based on location and/or the date of your request.

Cancer Care Ontario has closely followed industry standard security measures in the development and operation of the Screening Activity Report.

The information contained in the report is highly sensitive and contains personal health information. The disclosure of personal health information to family physicians is permitted under Personal Health Information Protection Act (PHIPA), 2004.

We have a robust privacy program in place to protect the personal health information that we collect, use and disclose for the purpose of cancer screening. Read more about Privacy.

In handling data from First Nations, Inuit and Métis (FNIM) sources, we respect the First Nations Principles of Ownership, Control, Access and Possession (OCAP) or Métis collective and self-determined data management and governance principles or Inuit data governance principles, as appropriate.

How Physicians and Nurses Should Protect Patient Information

  • All users should remember that the report contains personal health information, which must be kept confidential at all times.
  • User names and passwords must never be shared.
  • The site must not be accessed from non-secure computer systems, such as a computer in a public library or cyber café.
  • Protecting the privacy of patient information is a shared responsibility. Once the report is downloaded, users are responsible for protecting the information on their systems. This means, at a minimum, keeping computers up-to-date, running anti-virus software and scans, and using encryption.
