Apply to be a Patient and Family Advisor

Becoming a patient and family advisor or advisory council member at Cancer Care Ontario is a 3-step process:

Step 1

Call or email us to request an Expression of Interest form.

Submit the completed form by email, secure fax or mail:

Step 3

If your application meets our current needs, we will schedule a brief telephone interview with you to discuss your interest. If you don’t receive a response from us – don’t worry – we will keep your information on file until a relevant opportunity becomes available.
