Figure Description – Referral Pathway for Screening in the High Risk OBSP

The flow chart describes different referral pathways for breast cancer screening for women at high risk, and actions and decision points for physicians, the Ontario Breast Screening Program and genetic clinics.

  1. Physician assesses eligibility for the High Risk OBSP
  2. Physician answers question: Is the patient eligible for the High Risk OBSP?
    • If yes or maybe, refer patient to a High Risk OBSP site. Complete Category A or Category B of the OBSP High Risk Requisition form. Then proceed to step 3.
    • If no, discuss risk appropriate breast screening with the patient.
  3. OBSP site processes requisition.
  4. OBSP site answers question: Is the patient eligible for the High Risk OBSP?
    • If yes (i.e., Category A), the High Risk OBSP site books the screen.
    • If no, physician discusses risk-appropriate breast screening with the patient.
    • If maybe (i.e., Category B), the High Risk OBSP site refers the patient to a genetic clinic for genetic assessment. Proceed to step 5.
  5. Genetic clinic completes genetic assessment.
  6. Genetic clinic answers question: Is the patient eligible for the High Risk OBSP?
    • If yes, the High Risk OBSP site books the screen.
    • If no, the physician discusses risk-appropriate breast screening with the patient.
