Primary Care Quality Improvement Toolkits

The quality improvement toolkits are a resource for primary care providers who are required to submit Quality Improvement Plans to the Ministry of Health and Long-Term Care to plan, implement, monitor and report on quality improvements.

Why the Toolkits Were Created

Ontario's Excellent Care for All Act highlights a focus on quality in primary care. An essential element of Ontario's healthcare transformation agenda is the introduction of Quality Improvement Plans to the primary healthcare sector. The toolkits were designed by Cancer Care Ontario to help primary care practices implement Quality Improvement Plans in their practice.
