Follow-Up Model of Care for Cancer Survivors: Recommendations for the Delivery of Follow-up Care for Cancer Survivors in Ontario

Guideline Objective

These Recommendations aim to provide guidance to healthcare providers and administrators on implementing optimal delivery of follow-up care for all cancer survivors by clarifying:

  • The roles of primary care providers and specialist teams;
  • Settings in which this care should be provided; and,
  • Processes involved in organization of follow-up care.
