YOUR MAMMOGRAM RESULT

Dear Jane Sample:

On behalf of Ontario Health (Cancer Care Ontario), I would like to thank you for taking care of your health and having a mammogram. Your most recent mammogram, completed in OCT/21, did not show any signs of breast cancer.

If you have any questions, contact OBSP SAMPLE SITE at 222.333.4444 ext. 1.

The OBSP recommends that people ages 50 to 74 have a mammogram every two years. However, after age 74, the benefits of regular screening are not proven.

Your health is very important and we encourage you to speak with your healthcare provider to make a personal decision about further screening. To continue screening through the OBSP, your healthcare provider will need to provide you with a referral. If you do not return for further screening, this will be your last letter from the program.

If you notice any changes in your breasts, please contact your healthcare provider.

As an agency of the Ministry of Health, Ontario Health (Cancer Care Ontario) provides cancer screening programs to the people of Ontario. For more information about our screening programs, please visit ontario.ca/cancercheck, or contact us toll-free at 1.866.662.9233 or at cancerinfo@ontariohealth.ca.

Sincerely,

Judy Linton, RN, B.Sc.N, M.H.Sc.
Chief Nursing Executive, Ontario Health
